💡 Uses & Benefits

• Effective for treating hypertension, either alone or in combination with a thiazide diuretic

  Provides renal protection in type 2 diabetic patients with proteinuria (albumin-to-creatinine ratio >300 mg/g), slowing progression of nephropathy 

• Reduces risk of stroke and cardiovascular events in hypertensive patients with left ventricular hypertrophy (LVH)

  Acts by selectively blocking angiotensin II AT₁ receptors, causing vasodilation, reducing aldosterone, and lowering blood pressure 

📋 Dosage & Administration

• Standard dose: 50 mg once daily, with or without food

  If blood pressure control is inadequate, consider 25 mg twice daily before increasing to 100 mg/day (once or twice daily)

• For patients on high‑dose diuretics or low intravascular volume, start at 25 mg once daily 

• In elderly (>75 years): start at 25 mg once daily; under 75 years, no dosage adjustment required

⚠️ Side Effects & Precautions

• Common mild side effects: dizziness, diarrhea, nasal congestion, cough, upper respiratory infection, fatigue, edema, chest/abdominal pain, nausea, headache, pharyngitis 

  Rare but serious risks: hyperkalemia, angioedema, renal impairment—especially in volume depletion or concomitant ACE inhibitors/NSAID use

• Pregnancy Category D: contraindicated in second and third trimesters due to fetal renal damage and increased neonatal morbidity/mortality; caution during breastfeeding due to unknown excretion 

🧬 Drug Interactions

• Rifampicin and fluconazole may reduce active metabolite levels

• Combining with hydrochlorothiazide potentiates antihypertensive effect

• Caution with potassium-sparing diuretics (spironolactone, potassium supplements) due to risk of hyperkalemia

• NSAIDs (like indomethacin) may attenuate antihypertensive effect and increase renal risk 

⚕️ Use in Special Populations

• Renal impairment: for creatinine clearance <20 mL/min or dialysis, start at 25 mg once daily

• Hepatic impairment: consider lower dose due to increased drug exposure
