  • Effective low-dose immunosuppression: Methotrexate functions as a dihydrofolate reductase inhibitor, reducing DNA synthesis in rapidly dividing cells. At once-weekly low doses, it modulates immune activity to relieve symptoms and slow progression of rheumatoid arthritis, juvenile idiopathic arthritis, and psoriasis. 

  • Antineoplastic utility in select cancers: Methotrax is also used in oncology for conditions such as gestational trophoblastic diseases (e.g. choriocarcinoma, hydatidiform mole), acute lymphoblastic leukemia, non-Hodgkin’s lymphomas, mycosis fungoides, breast and head-and-neck cancers—often as part of combination chemotherapy.

ℹ️ Key Precautions & Safety Notes

  • Strict weekly dosing: Methotrax must be taken only once weekly. Daily dosing can cause severe toxicity. Maintain consistent teaching to prevent overdose. 

  • Pregnancy contraindicated (FDA Category X): Use during pregnancy or breastfeeding is strictly contraindicated due to high risk of fetal abnormalities and toxicity. Avoid conception during and for at least 6 months after discontinuation. 

  • Mandatory monitoring: Requires regular monitoring of complete blood count, liver and kidney function. Watch for signs of hepatotoxicity, bone marrow suppression, mucosal ulcers, and immunosuppression. 

  • Drug interactions: NSAIDs, sulfonamides, penicillamine, trimethoprim-sulfamethoxazole, and folic acid supplements may increase toxicity or reduce effectiveness. Avoid high doses concomitantly and adjust folate supplementation as guided. 

  • Side effects profile: Common adverse effects include nausea, fatigue, mouth ulcers, hair loss, headache, abdominal discomfort, GI upset, dizziness, and rash. Serious but rare effects include hepatotoxicity, bone marrow depression, pulmonary fibrosis, and infections. 

  • Overdose management: In case of overdose or severe toxicity, leucovorin rescue (folinic acid) should be given promptly to mitigate effects. Acute ingestion in children can be life-threatening. Administration guidance: Ideally taken on an empty stomach for optimal absorption. Can be given with meals to reduce GI upset. Avoid milk-rich products at dosing time. Stay well hydrated
