ℹ️ Indications & Dosage

• Primary uses: Treatment of mixed anxiety-depression, melancholic/masked depression, apathy, menopausal mood disorders, and psychosomatic symptoms with anxiety. 

• Adult dosage: Typically 2 tablets daily (morning and at noon). In severe cases, the morning dose may increase to 2 tablets. Once stabilized, maintenance is 1 tablet in the morning. 

• Elderly: Start with 1 tablet in the morning to minimize side effects. 

⚠️ Contraindications & Safety Notes

• Do not use in:

  • Immediately post-myocardial infarction,

  • Untreated narrow-angle glaucoma,

  • Bundle-branch conduction defects,

  • During acute intoxications from alcohol, barbiturates, or opiates,

  • Within 2 weeks of MAO inhibitor therapy,

  • In excitable or overactive individuals, due to risk of overstimulation. 

🚨 Side Effects & Precautions

• Common effects (at recommended doses):

  • Transient restlessness or insomnia.

  • Rare extrapyramidal symptoms (due to flupentixol activation). 

• Drug interactions:

  • Additive CNS depression with alcohol, barbiturates, sedatives, opioids.

  • MAO inhibitors may trigger hypertensive crisis.

  • Co-administration with antimuscarinics, CNS depressants, thyroid hormones, or sympathomimetics may alter response. 

• Overdose warning:

  • Anticholinergic symptoms (from melitracen), rare extrapyramidal reactions. Use gastric lavage and avoid adrenaline; treat seizures with diazepam, movement issues with biperiden. 

• Pregnancy/lactation: Preferably avoided due to limited safety data. 

• Precautions:

  • Gradually taper if switching from sedative drugs.

  • Monitor patients at risk of self-harm or suicide due to mood activation in early therapy.

  • Avoid in CNS-suppressed individuals or severe organ dysfunction.
