ℹ️ Precautions & Safety Notes

• Infection risk: Increased susceptibility to serious infections (e.g. tuberculosis, bacterial, viral, fungal). Screen for latent TB and monitor clinical signs of infection during therapy

• Laboratory monitoring essential: Baseline and periodic CBC, liver enzymes, lipids. Do not initiate or continue if lymphocytes < 500/mm³, ANC < 1,000/mm³ or hemoglobin < 9 g/dL .

• Dose adjustments in impairment: For moderate/severe renal or moderate hepatic impairment, reduce to once‑daily 5 mg; avoid initiation if severe impairment present .

• Drug interactions: Avoid potent CYP3A4 inhibitors/inducers (e.g. ketoconazole, rifampin). Dose modification required if coadministered with CYP3A4 and CYP2C19 inhibitors 

• Pregnancy & lactation: Category C. Use only if benefit justifies risk; unknown excretion in breast milk .

• Contraindications: Not to be used with biologic DMARDs or potent immunosuppressants concurrently; hypersensitivity to the drug is a contraindication
